Job Responsibilities

• Conduct discharge summaries and medication reconciliations
• Participate in clinical staffing and care plan meetings
• Document weekly care plans
• Perform pre-admission screenings and documentation per CMS guidelines
• Prescribe narcotics and perform PAPE/H&P
• Specialized in PMR procedures including stroke, amputations, and TBI

Qualifications

• Board Certified or Board Eligible
• Connecticut License at time of submission
• ACLS certification
• DEA certification (verbal confirmation required at name clear)

Working Hours

• Monday to Friday from 8:00 AM to 5:00 PM
• No weekend shifts required
