Job Responsibilities
- Provide direct patient care and co-management for oncology patients.
- Obtain medical histories, perform physical exams, and evaluate conditions.
- Develop and implement medical care plans with the oncology team.
- Conduct consultations and follow-up visits, ensuring supportive care.
- Participate in multidisciplinary meetings, tumor boards, and team meetings.
- Cross-train for coverage in the Cancer Survivorship Clinic.
- Maintain CoC standards with leadership collaboration.
- Liaise between supportive care, oncology providers, and ancillary services.
- Manage crisis interventions and symptom-focused care.
- Order medications and diagnostic tests, and evaluate results.
- Coordinate patient follow-up and care referrals.
- Maintain accurate, timely medical records and documentation.
- Participate in policy development, quality improvement, research, and education initiatives.
Qualifications
- Certification as an Advanced Practice Nurse from an accredited Nurse Practitioner program.
- Valid license to practice in the State of Connecticut.
- Preferred: Two or more years in Oncology as an APRN and Specialty Palliative Care.
- Knowledge of anatomy, physiology, oncology, and palliative care.
- ACHPN certification preferred or within the first year.
- EPIC experience.
