Job Responsibilities
- Provide comprehensive pharmaceutical care to patients, including management of high-risk medications, anticoagulants, and dosing for various medical conditions.
- Collaborate with healthcare teams to optimize therapeutic outcomes and ensure patient safety.
- Conduct thorough medication reviews and adjust dosing regimens based on pharmacokinetic and clinical data.
- Respond to code situations and provide emergency medication management.
Qualifications
- PharmD degree with a minimum of 5 years of experience.
- Certifications: BLS, ACLS.
- Experience with inpatient/acute care and ability to provide specialized dosing and medication management.
Working Hours
- 10-hour overnight shifts, 7 on/7 off schedule, including weekends and holidays as schedule dictates.
Skills
- Expertise in inpatient/acute care pharmacy services.
- Proficiency in anticoagulant and cardiac dosing, geriatric and pediatric dosing, and therapeutic medication monitoring.
- Familiarity with psychotherapeutic management and high-risk medication handling.
- Competence in compounding, IV medication administration, and sterile techniques.
- Ability to utilize an automated carousel system and respond to neurologic and cardiovascular patient needs effectively.
