  • Setting: Outpatient Clinic / Hospital
  • Start Date: ASAP
  • Schedule: Variable Shifts – 4×10-hour shifts, mostly nights from 8:30 PM to 6:30 AM; occasional day shifts from 10:15 AM to 8:45 PM; rotating days, generally 8 on 6 off, including weekends
  • Patient Population: All Ages
  • Location: Ukiah, CA

Job Responsibilities

  • Verify and fill orders for medications in compliance with hospital policies and regulatory guidelines.
  • Monitor patient medication therapies to ensure safety, cost-effectiveness, and efficacy.
  • Document interactions for medical review and supervise support personnel.
  • Ensure optimal medication prescribing with respect to indication, dose, route, frequency, and drug interactions.
  • Conduct and manage daily reports on medication utilization, participating in rounds to identify improvement areas.
  • Participate in all pharmacy-related tasks such as clinical rounds and population-based medication management activities.

Qualifications

  • Graduate of a school of pharmacy; Doctor of Pharmacy (PharmD) preferred.
  • Minimum of one year's technical experience in inpatient clinical pharmacy preferred.
  • Active pharmacist license in the state of practice required.
  • Advanced Cardiovascular Life Support (ACLS), Basic Life Support (BLS), and Pediatric Advanced Life Support (PALS) certifications required.
  • Experience with EPIC required.

Working Hours

  • Variable shifts primarily at night (8:30 PM to 6:30 AM) with occasional day shifts (10:15 AM to 8:45 PM).
  • Generally 8 days on, 6 days off, including weekends.
  • Holiday coverage for Christmas Eve and Day can be expected.
