Job Responsibilities

• Conduct 5–6 home health assessments daily
• Perform annual risk assessments and review chronic conditions
• Escalate urgent findings to the patient’s PCP
• Conduct A1C tests if applicable; other screenings are left for the patient
• Utilize an iPad, blood pressure cuff, and lab kits for assessment

Qualifications

• Must be Board-Certified FNP or ACNP
• Active NJ license and NJ Medicare number required

Working Hours

• Must be available between 8:00 AM – 8:00 PM, scheduled for an 8-hour shift
• Includes a one-hour lunch break
• Schedules released 2–3 weeks in advance

Skills

• Strong communication and assessment skills
• Ability to perform independent evaluations and escalate care as needed
• Proficient in using electronic devices for patient assessments
