Job Responsibilities
- Review and verify medication orders for accuracy, appropriate dosing, allergies, interactions, contraindications, and therapeutic effectiveness.
- Dispense medications in accordance with physician orders, hospital policies, and state and federal regulations.
- Provide clinical recommendations on medication selection, dosing, monitoring, and adjustments.
- Monitor patient medication therapies and relevant clinical information.
- Participate in medication reconciliation and support safe transitions of care.
- Prepare, verify, and oversee sterile and non-sterile medication compounding.
- Oversee and guide pharmacy technicians and support staff.
- Respond to medication-related inquiries from healthcare professionals and patients.
- Participate in emergency response situations as required.
- Ensure compliance with regulations, medication storage requirements, and hospital policies.
- Assist with medication inventory management and formularies.
Qualifications
- Preferred experience with inpatient pharmacy operations and automated dispensing systems.
- Experience with sterile compounding, IV medications, and antimicrobial stewardship.
- Familiarity with medication reconciliation and electronic medication-management systems.
