Job Responsibilities
- Review and verify medication orders to ensure accurate drug selection, dosage, and administration.
- Evaluate patient profiles for allergies, drug interactions, and lab results, and perform pharmacokinetic dosing for high-risk medications.
- Oversee the compounding and dispensing of specialized medications including IV preparations and chemotherapy.
- Participate in medical rounds, providing evidence-based therapeutic recommendations and supporting in emergencies.
- Conduct medication reconciliation at admission and provide drug counseling at discharge.
- Supervise and guide pharmacy technicians, interns, and students.
Qualifications
- Minimum of 2 years of inpatient pharmacist experience required.
- Ability to work in an Anticoagulation Clinic and perform Oncology compounding.
- Active Michigan pharmacy license.
