Job Responsibilities

• Conduct 5-6 home health assessments per day
• Perform annual risk assessments and review of chronic conditions
• Escalate urgent findings to the patient’s Primary Care Provider
• Perform A1C tests when applicable
• Utilize provided equipment such as iPad and blood pressure cuff

Qualifications

• Must be Board-Certified Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (ACNP)
• Active NJ license and NJ Medicare number required

Working Hours

• Must be available for 8:00 AM – 5:00 PM or 9:00 AM – 6:00 PM shifts
• 3-4 shifts per week with one-hour lunch breaks
