St. Peter's Health Partners is looking for Hospice PhysicianPractice Setting: Hospice Home CareReason for Coverage: Retirement and Home CareDates of Coverage: ASAP until indefinitelyCall Schedule: Call is on average 1 weeknight per week from 4pm – 8am and 1 in 4/5 weekends from 4pm on Friday until 8am on MondayCall Ratio: 1 in 4/5Average Number of Patients Per Day: Variable. The majority of the hospice physician's time is spent in interdisciplinary team meetings, reviewing cases for hospice eligibility, and providing guidance and recommendations to the nurses in the field. Home visits for face to face encounters and symptom management are scheduled as needed in accordance with the hospice physician's schedule on average 0-2 per day.Documentation System/EMR: EPIC – Comfort/Hospice ApplicationProvider Must be local – only mileage is reimbursed**Trinity Health does not reimburse for credentialing costs or application fees.**
